State Mandate for Insurance Coverage of Fertility Services
No mandate exists for both male and female fertility services, with the exception of patients who are enrolled in an HMO- health maintenance organization (HMO penetration was less than 1 in 3 residents in Ohio in 2016 per the Kaiser Family Foundation). For Ohio residents enrolled in an HMO, the insurance coverage mandate is quite vague. In practice, diagnostic testing (such as semen analysis, hormone testing, ultrasounds, and diagnostic procedures for testicular failure) are often covered for HMO patients. However, actual treatments (such as IVF, reversal of elective sterilization, and sperm extractions) are generally not covered.
